Membership Application

MBERSHIP CATEGORIES

Active Member:  An active member shall be licensed to practice Medicine, will reside in the State of Washington, and shall have completed formal residency training in dermatology.  A candidate must submit an application for membership to the Association. 
Associate Member:  An active member shall be licensed to practice Medicine, will reside OUTSIDE the State of Washington, and shall have completed formal residency training in dermatology.  Associate members are not eligible to hold an elected office or vote.  A candidate must submit an application for membership to the Association. 
Affiliate Member:  An affiliate member may be granted to those persons who are certified physician assistants.  Payment of regular annual membership dues shall be required. Affiliate members are not eligible to hold an elected office or vote.
Emeritus Members:  Members who retire from all remunerative active practice and have been a member of the Associate for more than 10 years, may request emeritus status.  The Association’s Board of Directors may upon application, grant a member who is incapacitated for health reasons and for this reason is unable to engage in active practice.  Emeritus members are not eligible to vote or hold office.
